Athlete Screening Forms

Each athlete screening form contains the following features:

  • Discipline Title at the top of the form

  • One dot per station at the discipline

  • Data collection

  • Navigation buttons

Users may navigate between forms by selecting the dots at the top of each page. The colors filling in each dot correspond to the status of data collection on a given form.

  • Red Dot - indicates the screening that the user is currently on

  • White dot - indicates a screening where no data has been collected

  • Black dot - indicates a screening where there has been data collected. Note that this does not indicate that all data fields have been completed; rather it indicates that some, potentially all, data fields have been saved.
